Program Registration

Step 1 – Become a Member of Kaleidoscope Kids

Our Membership Form covers all of the information we need to register your child for our programs, and you only need to fill it out once, updating the information as needed.  Best of all, it can be filled out on your computer using Adobe Acrobat or Acrobat Reader and then printed out or e-mailed directly to us.  E-mailed forms will require a signature in person prior to commencing the first registered program, unless digitally signed.

There is a one-time Membership Fee of $10, and this fee helps cover the cost of processing memberships and maintaining our database.

Financial Assistance Program

If you are seeking financial assistance in paying for our programs, please download and fill out our Financial Assistance Application.  This application will need to be filled out once per year.

Winter-Spring 2012 includes the 2012 March Break Day Camp

Please note:  If you need to make arrangements to pay in instalments, please contact us.  We will accept post-dated cheques and credit card pre-authorization, with a minimum deposit.

Step 3 – Submit Your Registration (and Membership) Forms to Us!

By Mail

Fill it out and return it to us with a cheque or money order made out to Kaleidoscope Kids of Toronto, or credit card information.

By Fax

If you’re paying by credit card or requesting a subsidy, you can fax your forms to 416-466-7236.

In Person

Registration forms can also be dropped off at our program space, 60 Bowden Street, on Saturdays between 10:00 am and 2:00 pm. Payment may be made by cash, cheque, money order or credit card. Forms and payment can be dropped through the mail slot in the door, but we do not recommend putting cash through the mail slot.

We accept Visa and MasterCard.
